Care Collaboration with non-VA Providers and Health Care Organizations

VISN 1 has obtained a grant from the VA Office of Rural Health to develop and implement systems and materials to improve collaboration between the VA and non-VA providers and health care organizations providing co-managed care to Veterans. 

A series of presentations to non-VA providers, case managers, VNA's and others is underway to enhance their knowledge of the VA health care system and to foster an improved working relationship. During the presentations, background information about VHA is presented, as well as information on transferring patients to a VA facility, obtaining VA records, obtaining medications and supplies, and what the VA does and does not pay for when a Veteran is at a non-VA facility.

The files listed below are the Power Point presentations being used by VISN 1 facilities when they present to non-VA provider groups. Those files with a medical center name have been customized to reflect the local processes and contact information for that medical center, and the Standard Presentation is the template on which the facility presentations are based.
